About the Role As a Support Worker, you’ll play a vital role in supporting clients to participate in social, community and vocational activities, promoting independence, dignity and overall wellbeing. You’ll work as part of a warm, collaborative team, primarily based at the main CBD centre, with some community outreach work required from time to time. Key Responsibilities Delivering individualised, person‑centred support in line with client support plans Supporting clients with daily activities, social engagement and group programs Assisting with personal care, hygiene, nutrition and wellbeing where required Assisting with transporting clients to and from the centre and community activities Facilitating activities that build confidence, skills and enjoyment Supporting daily dining room activities and understanding dietary needs Assisting with light cleaning duties and providing back‑up support in the kitchen when required Maintaining accurate documentation, file notes and reports Working in line with Aged Care Quality Standards, WHS and organisational policies About You You’re a calm, capable and compassionate Support Worker who brings professionalism and heart to your work. You thrive in a team, but are confident working independently, and you genuinely enjoy supporting people from diverse backgrounds. To be successful, you will have: Certificate III in Individual Support (or equivalent) Demonstrated experience supporting older people and/or people with a disability A strong understanding of person‑centred, holistic care A current Working With Children’s Check (or willing to obtain) A current First Aid Certificate Food Safety Certificate (or willing to obtain) Current Class C Driver’s Licence Intermediate computer skills and experience operating MS Office Suite Excellent communication and documentation skills A respectful, patient and non‑judgemental approach Willingness to undergo a Police Check and meet vaccination requirements Experience working with clients with dementia will be highly regarded.
